Congratulations on being part of your employer’s group health plan that includes a direct primary care (DPC) option to register yourself and your family for direct primary care!
Your employer will email you a link that will take you directly to the enrollment page at the time of registering. You will need names, birth dates, cell phone numbers, emails, and addresses for yourself and any dependents.
Please register before the 25th of the month in order for your benefits to start the first of the following month.
